1. Must bring your child\'s current THS (Medicaid) card to each appointment
2. When you call to make an appointment, let the clinic know that your child has THS (Medicaid)

Children from 1 to 18 years of age

1. Must bring your child\'s CHIP card to the first appointment
2. When you call to make an appointment, let the clinic know that your child has CHIP

Adults and Children

1. Must bring proof of income (check stub, income tax form, notarized letter from employer)
