Their staff will take a sensitive, compassionate approach to your Oral Health Care and seek to provide pain-free dental care services.
Hours:
Clyburn Center- Adult Medicine & Pharmacy:
*Monday-Thursday 8:00am-7:00pm.
Intake Process:
*Call for an appointment (preferred), or*Walk-in.
Bring:
*Picture ID,*Social Security card, and*medical/psychiatric medical records.
*For low income discount, bring ID and last month's pay stubs and proof of household size.
Program fees:
*vary. *Sliding scale available. *Agency accepts Medicaid, Medicare, private insurance, cash, checks, and credit cards. *Agency will not turn anyone away that may not have the ability to pay if they have chronic conditions.
