New patients must supply birth certificates and immunization records for children. If new patient applying for sliding scale fee, bring 3 recent pay stubs or a notarized letter from employer. Program Fees:Accepts most private insurance, Medicaid, Medicare, VA coverage, low income, uninsured. Sliding fee discount available for qualified patients.
