The dental clinic will be on the first 3 Mondays of each month

* 6:00pm-8:00pm

Intake Process:

First come, first serve.

Please bring:

*Valid photo ID

* Proof of Residency (Phonebill, utility bill, etc.)

* One of the following: 1099, 2 recent pay stubs, tax return or food stamp letter
