The Clinic provides no-cost services in dental extractions, restorations (fillings) and hygiene (cleanings).

It is the decision between the Doctor and the patient as to how many extractions, fillings and cleanings can be performed in one sitting.

Sometimes it is in the best interest of the patient to do half now and the other half at a later date.

PPH&D Clinic always tries to save the tooth.

In some situations, this may not be possible and the Dentist will educate the patient as to the best treatment plan and why the Dentist suggests this.

At Park Place Health and Dental Clinic Inc, we take care to provide their customers high quality services personalized for their unique needs.

They are available to meet your dental needs during the set Office hours.

Their Dental Staff consists of local Volunteer Professional dentists, hygienists, Dental Assistants and Nurses who provide excellent care.

What YOU need to bring to the Eligibility ScreeningPPH&D Clinic uses an Eligibility Screening system that will take no more than 15 - 20 minutes once you get into seeing the screeners.

Eligibility Screening is on Mondays starting at 8 AM.

This involves coming to the Park Place Multi-Purpose Center and then sitting in the lobby and going through the screening process.

Appointments are on a first come, first serve basis.

Eligibility screenings can occur during the rest of Mondays and early Tuesdays but the number of available appointments gets limited as Monday and Tuesday fills up.

Remember appointments cannot be made by phone and are first come, first serve.

There are Four Very Important Items that You need to bring with You to the screenings.

First, you have to be a Norfolk resident to be seen at this Clinic.

You need to bring Proof of Norfolk Residency.

This could be a lease with your name and address on it.

It could be an official bill that has your name and local address on it with in the last thirty days.

This needs to be a very recent bill with your name and address on it.

If you live at a shelter, you need to get a letter stating that you are a resident of that shelter.

Second, you will need to bring Proof of Household Income.

The PPH&D Clinic serves the homeless, low-income and under-insured of the city of Norfolk.

Because of this you will need to provide documentation that shows that the household does not earn more than 125% of the Poverty level.

This amount varies according to who are actually living in the household as members of the family.

You will need to bring your two most recent pay stubs from within the last thirty days, or a SS Letter of monthly benefits or some other official document stating what each adult person in the home makes.

The total earned income from all members of the household is used to determine eligibility.

Please contact the dental clinics themselves to confirm that the hours and/or services have not changed.
