Sliding fee scale patients are those with household incomes that fall at 100% and below poverty up to 200% of federal poverty.
Hours:
Monday and Thursday 8:00 am - 5:00 pm.:
Tuesday 8:00 am - 7:00 pm;
Friday 8:00 am - 2:00 pm.
