For patients who do not have dental coverage, visit fees will be based on family size and household income, according to a sliding fee scale.

Upon check-in the client will be responsible for paying fees based on the scheduled services to be preformed during the dental appointment (i.e. procedures, exam, x-rays, etc.) based on the sliding fee scale.

Please bring the following information with you to your appointment to make the registration process go faster:

1. Proof of income (required to qualify for discounted sliding fee scale).
2. Examples are pay stub, tax returns, or a letter from employer.
3. Copy of insurance card (Medicare, Medicaid, private insurance)
4. All medications you are taking (prescription, herbal, over the counter)
5. Address verification (utility bill, phone bill, etc.)
