This clinic provides the following services:, Chicago Family Case Management Program, Childhood Literacy - Reach Out and Read, Deaf Access Program, Dental Services, General Health Care Services, HIV Services, Immigration Physicals, Low-cost Breast & Cervical Cancer Screening, Ob/Gyn and Midwifery, Services for People with Disabilities, Specialty Care, Substance Abuse Services, Westside Healthy StartACCESS provides a national model of quality, affordable community health care for medically underserved populations that are tailored to meet the specific health needs of the individuals, families and communities we serve. Cost: Medicaid, Medicaid HMO, Medicare and most major health plans are accepted to cover the costs of medical care. A sliding fee scale based on ability to pay is available for uninsured patients. Case management services are provided at no charge. Hours: Monday, Wednesday & Friday, 8:30 am - 5:00 pm Tuesday, Thursday, 8:30 am - 7:00 pm Saturday, Closed
