Eligibility requirements:

• Adults who are defined as eligible under federal poverty guidelines.
• Must have referral from MHS Primary Care PRogram

Preventive and limited routine dental care for eligible adults that are referred to the clinic through the Memorial Health Care System Primary Care Program.

The Primary Care Physician makes a referral based on pre-established citeria.

Services provided:

• Dental and Healtcare.
