Children ages 3-20 must have one of the following insurances:

• Medicaid,
• Sunshine State Health Plan,
• First Coast Advantage,
• United Healthcare Medicaid,
• CMS/Pedicare, or Dentaquest through Kid Care.

There is also a Children\'s Dental Bus available the first week of every month at  Cherry Elementary School located at 420 West Edson Drive, Orange Park, FL 32073.
