Adjustable Payment Plan (APP) has been developed to provide financial assistance for patients who are uninsured, underinsured and/or have been denied health insurance.

Eligibility for this Plan is based on gross family income (all working members within the household), number of family dependents and is in compliance with Federal Poverty Guidelines as published annually.

Sliding Fee Scale Hours: 

• Monday Tuesday 8:00 am 6:00 pm
• Wednesday - Friday 8:00 am 5:00 pm
