Restorative dental procedures are offered to homeless patients who are:

1. completing addiction recovery
2. vocational training
3. or job-skill enhancement programs.

For the uninsured who are not homeless, this clinic offers a sliding fee discount, based on your family size and finances.

You will need:

• a pay stub
• an unemployment check stub
• a recent tax return
• or a letter from your employer to prove your family income to qualify.
