Sliding Fee Scale Plan - A health care discount for people who do not have insurance and who do not qualify for AHCCCS. Services provided are: 1. Oral exams and x-rays 2. Preventive treatments - oral cleaning, prophylaxis, topical fluoride treatments, sealants 3. Emergency treatment for oral/dental pain and infection 4. Restorative treatment - fillings, crowns 5. Basic endodontic treatments - pulp cap and simple root canal therapy 6. Basic oral surgery - simple extractions, incision and drainage and biopsy 7. Basic periodontal treatment 8. Dentures - denture relines and adjustments 9. Prescribing necessary medications 10. Education and follow-up services 11. Making appropriate referrals to specialist
