Provides healthcare for persons lacking housing and persons residing in supervised facilities or who are residents of transitional housing.
Provides medical care, medications,dental care,vision care, outpatient detoxification and social services to homeless individuals and their families.
RN on duty sets appointments based on severity.
Transportation is available to and from appointments.
No one is refused due to inability to pay.
Eligibility:
Must be homeless or a resident of a transitional housing program.
Persons remain eligible for up to one year after they have found permanent housing.

Adult Medicine & Pediatrics:
8:00 am-7:00 pm, Mon-Fri, 9:00 am-1:00 pm,
Sat Dentistry:
7:30 am-9:00 pm, Mon-Fri, 9:00 am-3:00 pm,
Sat Optometry and Radiology:
8:00 am-5:00 pm, Mon-Fri
Pharmacy:
8:00 am-6:30 pm, Mon-Fri, 9:00 am-1:00 pm, SatNo fees for service.
