Medical and Dental Hours: Monday through Friday 9-5 p.m. No patient will be declined care because of an inability to pay for services. Patients will be expected to comply with the efforts of the Sunshine Clinic staff to ascertain the existence of any third-party insurance (Medicaid, Medicare, private insurance, SCHIP, etc.) coverage a patient may possess, or otherwise appropriately document said patients inability to pay for services. Sunshine Clinic does not want financial constraints to prevent individuals and families from seeking the care that is needed; therefore we have adopted a sliding fee discount schedule. Patients qualifying for a sliding fee discount will be expected to pay an affordable flat fee at the time services are rendered. This payment will cover a single provider visit. There will be an extra minimal charge for ancillary services such as lab and x-rays. This payment does not cover pharmaceuticals or dental, there is a separate schedule for those services.
